How to Complete the DOT Employer Background Check
As part of our regulations under the Federal Transit Authority (FTA), UZURV is required to conduct a previous Department of Transportation (DOT) employer background check. This is to determine if you have been subject to any DOT employment-related drug and alcohol screenings.
You will submit your work history information for any DOT-covered employment you have had during the last two years. This does not include UZURV or any other rideshare company.
Your information will be submitted through the DISA Global Solutions portal to initiate the check.

You have 14 days from the time you receive your invite link to submit your information to DISA Global Solutions. After 14 days, you’ll need to contact support@uzurv.com to get a new link.
Yes, I was covered under the DOT
What you need:
- DOT Employer Name and Phone
- Employment Dates
- Identification Document (optional)

Acceptable Identification:
- Driver’s License
- Passport
- Birth Certificate
- Social Security Card
How to submit your information
1. Tap the access link in your DISA email invite to register.

2. Read disclosures and provide your electronic signature to authorize the check.
3. Add your personal information and current address.
4. Add your DOT work history.

5. Provide one form of identification. (optional)

6. Tap ‘Next’ to submit your information.
7. If you have a Commercial Drivers License (CDL), DISA will run an additional check in the FMCSA Clearinghouse.
You will receive additional instructions from DISA that requires your consent. Look for an email from DISA with “FMCSA Clearinghouse” in the subject line.
No, I was not covered under the DOT
1. Fill out this form, indicating that you were not covered under a DOT employer during the past three years.
2. If you have not done so already….

- Fill out the form and sign it in ink
- Upload the form to your app
NOTE: UZURV will alert you with further instructions if needed. You may not receive a notification from DISA regarding your results.
DISA Global Solutions FAQs
The Department of Transportation (DOT) requires UZURV drivers to verify their work history. This is to ensure they passed any DOT-administered drug or alcohol screening within the last 2 years.
If you have a Commercial Drivers License (CDL), DISA will run an additional check in the FMCSA Clearinghouse.
You will receive additional instructions from DISA that requires your response. Look for an email from DISA with “FMCSA Clearinghouse” in the subject line.
If you were covered under the DOT, DISA will contact your previous employer to verify that you passed your screenings.
Please note that only DOT employers will be contacted by DISA for verification.
Submitting your work history and authorization takes 10 minutes using your phone or computer. DISA’s verification process typically takes 3-5 business days. However, it can take much longer if DISA cannot reach your previous DOT employer.
You have 14 days to submit your information from the time you receive the DISA email access link. This is why it is important to submit your work history as soon as possible.
