DC, MD, and NoVA Onboarding Guide
This guide outlines documents, signatures, and certifications you need to become a UZURV Driver in the DC Metro, Maryland, or Northern Virginia.
To see next steps, check each location where you plan to drive.
(The D.C. Metro area includes D.C., Northern Virginia, Montgomery County and Prince George’s County.)

Earn up to $200 in bonuses.
Onboarding can take up to a few weeks. This process sets you up for success, and the time you take to onboard is worth it.
You’ll earn good money helping others driving on UZURV’s platform. Plus, startup bonuses:
- $50 bonus when you finish onboarding and take your first trip.
- Another $50 after 5 trips
- Plus another $100 after your first 30 trips.
That’s $200 in startup bonuses and good work that makes a difference.

Choose your Transit Programs
Next, choose your transit programs so that you complete the correct requirements to become a UZURV Driver.
Transit programs represent UZURV partners. They determine the rides that show in your Driver Feed.
If you ONLY want to drive in the D.C. Metro Area, make sure the following programs are selected in your app:
- Alexandria DOT ADA Supplemental
- Arlington ART STAR
- WMATA: DC, NoVA, Montgomery, Prince George’s
Depending on where you live, the Drive app preselected transit programs for you. You may need to remove programs in areas you do not wish to serve.
IMPORTANT: In the future, should you wish to drive in other areas, you will need to complete requirements for any transit programs you add back into your app.
If you ONLY want to drive in Baltimore and Anne Arundel County, make sure the following programs are selected in your app:
- Anne Arundel County Rider Choice
- Baltimore MTA Mobility Link
- MTA Call A Ride
- MTABALFT: Greater Baltimore

Complete Your Documents
Go to the Documents tab for your transit program. Tap on each document to upload it to the app.

All UZURV drivers are required to have CPR and First Aid Certification before they can drive. If you don’t already have one, you can easily get a free certification online through our partner, NewLife CPR.
Get your CPR/First Aid certification online now. Sign in with code DRIVEUZURV to ensure it’s free.
After you finish your certification or if you are already certified, upload a picture or screenshot of your certificates to the Drive App. Click here to upload in the app.
The Department of Transportation (DOT) requires all drivers to:

- Acknowledge they passed any DOT-administered drug or alcohol screening taken within the last two years.
Download the DOT Notification and Acknowledgement Form using the button below. Fill it out and upload the completed form to your Drive app.
Baltimore and Anne Arundel County Drivers must submit a yearly MD Vehicle Safety Recall Form, stating that your vehicle doesn’t have open safety recalls.
Here’s how to submit yours:
- Download and fill out the Vehicle Safety Recall form here. You must download and complete a form for every vehicle you plan to drive with UZURV.
- On the card, sign and enter the date you submit the form.
- Take a photo of the completed form and upload it to the Drive app.
- In the app, enter the expiration date. Expiration is one year from the date you submit the form. (For example, if you submit your form on July 17, 2024, use July 17, 2025 as your expiration date in the app.)
UZURV Drivers in Baltimore and Anne Arundel County must complete a one-time fingerprint background check.
You must complete the fingerprint background check and provide all required documents (fingerprinting receipt, recall attestation, vehicle inspection, insurance, etc.) before we can issue your screenings and standard criminal background check. These are two different background checks and both are required.
Here’s what you need to do:

- Go to the Maryland Fingerprint Background Check Locations document.
- Choose a location for your fingerprinting.
- Download and fill out the pre-registration application for the location you choose. Make sure you download the correct application.
- Schedule an appointment.
- Take the completed application and your ID (valid drivers license or passport) with you to your appointment.
- After fingerprinting, upload a photo of the MD Fingerprinting Receipt to the app within 45 days of completion.
NOTE:
- You DO NOT need to upload your pre-registration application.
- If you also plan to drive in Arlington County, you must submit a Criminal Conviction Disclosure Form (CCD Form) and get an Arlington fingerprint background check in addition to the Maryland Fingerprint background check.
All Arlington drivers are required to take a fingerprint background check.
Before scheduling your fingerprinting appointment, you must submit a Criminal Conviction Disclosure Form (CCD Form). Open and complete the document, then take a screenshot and submit it to the app.
You will be instructed how to schedule a fingerprint appointment once you begin your UZURV certifications.
NOTE: If you plan to drive in Baltimore or Anne Arundel County, you must also submit a fingerprint background check in addition to the Arlington fingerprint background check.
All UZURV drivers must provide proof of auto insurance. The document you upload to the app must have the following information:
- Name
- Vehicle Information (Make. Model, Year, VIN#)
- Effective Date in Month/Day/Year format
- Expiration Date in Month/Day/Year format. (We cannot accept “expires one year from effective date.”)
Please note: We cannot accept a Digital ID, Apple Wallet, or app version of the auto insurance ID card. It must be a PDF, screenshot, or photo of your auto insurance ID card or declaration page.
For vehicles registered in Washington, D.C., we accept any DC DMV Inspection completed within the last two years. You may provide an inspection certificate or sticker for D.C. inspections only.
For vehicles registered in Virginia, we accept any Virginia State Inspection completed within the last year.
For vehicles registered in Maryland, Drivers are required to have a current, valid Maryland Safety Inspection (MSI). We accept any verifiable MSI Certificate completed within the last year. If you have a valid MSI, upload a picture of the form to the app.
Here’s how to schedule a free Maryland Safety Inspection if you need one:

- Pep Boys
- Schedule your inspection with one of these Pep Boys MSI-certified locations.
- At your appointment, tell them you are with UZURV. Then provide the Fleet Code: 80537426 and Part Number: PN4836. Your inspection is free ONLY if you provide the fleet code and part number.
- AAA
- Schedule your inspection with a AAA location near you.
- At your appointment:
- Tell them you are with UZURV. Show this letter and your UZURV Drive app profile (You may show either document on your phone or bring a printed copy of the letter.)
- Provide the UZURV Account Number: #9998108180 and tell them you want Service Type: #17. Your inspection is free ONLY if you provide the account number and service type.
- Pep Boys
IMPORTANT:

- The only way to receive a free MSI is through Pep Boys or AAA. Drivers who complete an MSI at another station are responsible for the cost.
- UZURV does NOT reimburse for repairs or replacement items required to pass the inspection. AAA offers a discount if you need repairs. For discount, use UZURV Account Number: #9998110309, Service Type: #18.
- Drivers who submit fraudulent inspection documents will be removed from the platform and will be ineligible to become a UZURV Driver.
Provide Your Signatures
When you tap Signatures, here’s what you’ll see:

Click on each document to sign your:
- Partner Agreement
- Terms of Service
- Form W9 – be sure to enter your full legal name
- Background Check Consent Form
- Screening Disclosures and Authorization
- And any other documents listed
You must sign documents on your phone in landscape mode. Here’s how:
- Tap the document to sign.
- When you see the 3 dots, turn your phone sideways. A signature screen will appear.
- Sign the document, using your finger or a stylus.
If the signature screen does not appear when you turn your phone sideways, check your phone’s orientation lock in Settings and disable it. Then try signing documents again as described above.
STOP!
Do not move on until you’ve uploaded all required documents and provided all required signatures.
Complete your Screenings
Once all documents and signatures are reviewed and approved, we send you a screening package via text message and/or email. You will use the app to complete this. These screenings include….
- Criminal Background Check
- Motor Vehicle Record (MVR)
- Department of Transportation (DOT) Drug Test
Please note: If you plan to drive in Arlington, Baltimore, or Anne Arundel County, you are required to pass a criminal background check in addition to the fingerprint background check. These are separate requirements.

The minimum requirements to drive include:
- You must be at least 21 years old
- You must have a valid US Driver’s License from your state of residency
- You must be able to communicate well using the English language
- You must be able to pass a….
- Criminal background check (past 7 years minimum)
- National Sex Offender Registry background check
- Motor vehicle background check
- Drug screening
This program may have additional requirements. See the yellow highlighted section below for the full list of requirements.
Your vehicle must….
- Have 4 doors, like a sedan, SUV, or van (No pickups!)
- Be 2015 model year or newer (2018 for Phoenix and Dallas)
- Safely seat 4-7 passengers with working seatbelts
- Be registered for personal use only. No rental cars, short-term leases, commercial vehicles (such as taxis, liveries, etc.)
- Not display a vehicle wrap or other commercial signage
- Not have a salvage or rebuilt title
- Have valid vehicle insurance (personal, not commercial) in your name and from your state of residency
- Have a valid vehicle registration from your state of residency
- Pass a vehicle inspection, as required by your transit program, regardless of state inspection requirements.
- Be in good working condition with no cosmetic or major damage
- Be clean, tidy, and a NO SMOKING vehicle
If you want to a vehicle with wheelchair access modification (lifts, ramps, etc.), you’ll need to apply here as there may be additional requirements.
A transit program represent a UZURV partner that determines what rides show in your Driver Feed. Depending on where you live, the Drive app preselects transit program(s) for you that provide rides near you.
If your app shows multiple transit programs, make sure you have the right transit programs for your driving needs before you start your document requirements.
Having the right transit programs means:
- You only complete the requirements you need. Nothing extra.
- Your app shows rides in the locations where you plan to drive.

UZURV Riders are people in your community who need additional assistance with mobility. The majority of our riders depend on everyday vehicles for transportation.
Our Riders are….
- People with disabilities
- Older adults
- Families with special-needs children
- People who rely on service animals to guide them and keep them safe
- Anyone with mobility limitations who need a little help getting to where they need to go
UZURV partners with hospitals, government agencies, insurance companies and nonprofits to provide a higher level of care to people with disabilities and older adults. In doing so, UZURV drivers must meet state TNC and Federal Transit Administration (FTA) regulations as well as requirements set by our program partners.
Additionally, our drivers need to be versed in the Americans with Disabilities Act (ADA) requirements, as well as the privacy expectations of the Health Insurance Portability and Accountability Act (HIPAA).
While it takes a little longer to complete UZURV’s onboarding requirements and credentialing, our drivers find earning ~30% more per ride than driving typical rideshare makes it well worth the effort!
IMPORTANT REMINDER: This program requires UZURV drivers to have at least 1 full, uninterrupted year to the month and day of verifiable driving history in the United States. If we cannot establish your driving history, we may request additional documentation.
- Baltimore drivers must not have a license suspension or revocation during the past 7 years.
- Minimum Driver Age is 21 years.
- Must have a valid U.S. Driver's license from your state of residency.
- You must be able to communicate well using the English language.
- No rental or short-term lease vehicles.
- Vehicle must have at least 4 doors. No pickups or commercial vehicles!
- Vehicle must have seating and seatbelts for 4 to 7 people.
- Vehicle must be 2015 model year or newer.
- Vehicle must be clean and free of damage or large exterior branding or advertisements.
- Must have valid vehicle insurance (personal, not commercial) and registration, both from your state of residency.
- Must not have failed a Department of Transportation drug screening in the past two years.
