How to Verify your Work Eligibility with Form I-9

Your transit program requires all drivers to complete Form I-9, verifying your eligibility to drive with UZURV. Follow the three steps below to complete this requirement.

How to submit your documents:

1. Fill out Federal Form I-9 to verify eligibility to drive with UZURV.

Only fill out the top portion (Section 1) of the form.

Then take a photo or screenshot of the completed form and upload it in your app.

2. Read, fill out, and sign an E-Verify Consent Form, allowing UZURV to verify your eligibility to drive with us.

3. Provide documentation you are eligible to drive with UZURV by taking a photo of ONE document from the list below and uploading it in your app under I-9 Supporting Document.

You only need ONE document from this list:

    • U.S. Social Security Card
    • Passport
      • U.S. Passport or U.S. Passport Card
      • Foreign passport that contains a temporary I-551 stamp or temporary I-551 printed notation on a machine-readable immigrant visa
      • Foreign passport with Form I-94 or Form I-94A with Arrival-Departure Record, and containing an endorsement to work
      • Passport from the Federated States of Micronesia (FSM) or the Republic of the Marshall Islands (RMI) with Form I-94 or Form I-94A indicating nonimmigrant admission under the Compact of Free Association Between the United States and the FSM or RMI
    • ID Card
      • U.S. Citizen ID Card (Form I-197)
      • Identification Card for Use of Resident Citizen in the United States (Form I-179)
    • Employment Authorization Document
      • Employment Authorization Document that contains a photograph (Form I-766). Please provide FRONT and BACK images of the document or card. 
      • Employment authorization document issued by the Department of Homeland Security (DHS). Please provide FRONT and BACK images of the document or card. 
    • Birth Certificate
      • Certification of report of birth issued by the Department of State (Forms DS-1350, FS-545, FS-240)
      • Original or certified copy of birth certificate issued by a State, county, municipal authority, or territory of the United States bearing an official seal
      • Original or certified copy of birth certificate issued by a State, county, municipal authority, or territory of the United States bearing an official seal
    • Permanent Resident Card or Alien Registration Receipt Card (Form I-551). Please provide FRONT and BACK images of the card. 
    • Native American tribal document

A receipt for a replacement of a lost, stolen, or damaged document may be presented in lieu of any above document for a temporary period.
