Driver Materials
Drivers must display the UZURV Trade Dress & any required materials for every ride
If your trade dress or materials are lost or damaged, complete the form below. Please complete a form for each vehicle that requires new materials.
- We’ll mail new materials at no cost.
- We’ll include instructions for displaying the materials. Your program’s Driver Hub also has placement instructions.
- Failure to display Driver materials will result in suspension and possible removal.
IMPORTANT: If you are a new driver, do not complete this form.
Your Welcome Kit containing your Driver materials was automatically mailed to you and will arrive in 7-10 business days. If you do not receive your Welcome Kit within 10 business days, contact DriverCompliance@uzurv.com.
UZURV Materials Order Form
To request new materials, complete this form and click Submit.
- Enter the email address that you use in the Drive app.
- Enter the address where you live. If your address is different from the address listed in the Drive app, please update it.
- Scroll to the area where you drive with UZURV. A materials list will pop up. Check the boxes next to the materials you need and submit the form.
- Your replacement materials should arrive within 7-10 business days.
- Required materials are free to active UZURV Drivers.
- If you are a new driver, do not complete this form. Your Welcome Kit with driver materials is in the mail.
NOTE: Suspended Drivers will not receive new materials. Contact DriverCompliance@uzurv.com for steps to take to reactivate your account.
