WAV Provider FAQs

WAV providers will need to maintain and be able to provide on-demand the following records:
  • Valid business License
  • Current W-9
  • Certificate of Insurance (COI)
  • Vehicle and driver log
  • Valid driver’s license for all drivers
  • FTA Drug and Alcohol Testing records for all drivers including collection and sample methods (pre-employment, post-accident, reasonable suspicion and random)
  • Motor Vehicle Records (MVR)
  • Criminal Background Check for all drivers
  • A WAV Provider Application
  • Interior and exterior photos of each WAV vehicle
  • Disadvantage Business Enterprise (DBE) Certificate – if applicable

UZURV WAV Drivers will need to be able to assist passengers from their door to the vehicle (door-to-door).

Drivers must also have completed proper compliance training and Providers must maintain all training records (see details in FAQ below).

Providers must use WAV vehicles that are no more than 7 years old (vehicle age requirements may vary by program).

UZURV WAV Providers will also need to maintain and provide proof of required insurance (see details in FAQ below).

You must have a signed completed contract with UZURV.

UZURV WAV Drivers must have the following training:
  • Safety and operational policies and procedures
  • Operation and maintenance of wheelchair lift and other special equipment including proper wheelchair securement procedures
  • Defensive driving
  • ADA sensitivity, passenger assistance, boarding, alighting and securement
  • Handling of incidents, accidents, emergencies and security threats
  • CPR and First Aid Certification
  • HIPAA
  • Drug/Alcohol Abuse
  • Passenger Assistance Safety and Sensitivity (PASS) or PASS equivalent training

Any certifications and training records must be documented by the WAV provider and available for review by UZURV.

Yes. All WAV Providers must maintain Commercial General Liability insurance with per occurrence limits of at least $1,000,000 and general aggregate limits of at least $2,000,000 in order to work with UZURV. They must also maintain Business Automobile Liability insurance covering owned, non-owned and hired motor vehicles with liability limits of at least $1,000,000 per occurrence. These insurance policies must be endorsed as primary insurance and not contribute to any other insurance maintained by UZURV.
WAV Providers must also maintain worker’s compensation and employer’s liability for its employees involved in performing services for UZURV.
WAV Providers’ insurance carriers and policy provisions must be acceptable to UZURV’s Insurance Manager and remain in effect for the duration of any Contract. UZURV shall be named as an additional named insured on the Commercial General Liability insurance, Auto Liability and WAV Provider’s insurance policies by endorsement. WAV Provider will cause any of its subcontractors, who provide materials or perform services relative to this contract, to also maintain the insurance coverages and provisions listed above.
WAV Providers must submit certificates of insurance in order to begin working with UZURV. The insurance certificate will indicate that UZURV will be given 30 calendar days written notice prior to the cancellation of coverage.
